Case Management
Our Adult Case Managers are all certified and will help you to identify services needed to assist you in your own personal recovery. 

CM Requirements 
  •  To receive Case Management, you must have:
        • a documented mental health diagnosis
        • Service needs that are either not met, or difficulty accessing services on your own

 

 

 

 


What will my case manager do 
  • Your case manager will meet with you, and together assess what services you are in need of.
 
  • Your case manager will also refer you to service providers to meet your needs, monitor those services to make sure you get what you need; and advovate for you.
 
  • Your case manager will visit regularly to encourage wellness initiatives such as socialization, exercise, and

                            good nutrition, key elements that support good health.
