Sober Supported Housing Admission Criteria
Communication & Cognitive Abilities:
· Residents must be able to express needs within reasonable limitations brought about by age and diagnosis
· Residents must be able to independently able to use the telephone
· Residents must be mentally alert and able to evacuate the home without assistance in case of an emergency
Activities of Daily Living and Medical Matters:
· Residents must be able to prepare meals or snacks without outside assistance, although the case manager may assist in obtaining meals on wheels, frozen dinners, etc..
· Residents must be able to take care of the living unit and safely use facilities such as kitchen range, tub/shower.
· Residents (with case manager assistance) must be able to take care of finances.
· Residents must be able to tend to personal needs such as toileting without reminders
· Residents must be medically stabilized, as there are no medical providers on site.
Safety:
· We complete a background check on all potential residents.
· We complete a urinalysis on all new admission, and we do not accept people who have substances in their system which are not prescribed, or not taken as prescribed.
